By applying for above anti-COVID-19 packages including disposable face masks and Chinese medicines (both are together called “Supplies” hereon), I hereby release all of the “Donor” including U.S. Hunan Association, its directors, officers, volunteers and original and current donors of Supplies from below liabilities:

1. No Warranty: I acknowledge that the Supplies have no warranty and hereby accept them “As Is” and accept the responsibility for any alleged defect, failure or issue of any type arising out of or in relation to the Supplies; and

2. Waiver of Liability: I’m releasing the Donor of all liability of any loss or injury to persons or property which may be sustained while using any and all Supplies given.

作为以上防疫物资爱心包(包括一次性口罩，以及/或银黄清肺胶囊，以及/或新冠肺炎预防2号方颗粒， 以下统称“防疫爱心包”)的申请/收件人， 我在此特别免除“捐赠单位”包括美国湖南联谊会并其全部理事, 负责人，义工和防疫爱心包物资原始捐赠者以下所有责任:

1. 无保证：申请/收件人接受捐赠的防疫爱心包物资的“原样”; 捐赠单位不提供任何保证； 我个人将负责使用过程中所发生的任何问题。

2. 放弃责任：可能因对捐赠的防疫爱心包物资使用造成的人身或财产伤害，申请/收件人特此放弃和解除对“捐赠单位”追究任何的责任。
